14TH SANCTIONS PACKAGE AGAINST RUSSIA PROHIBITING CERTAIN EUROPEAN ENTITIES FROM ACCEPTING FINANCING OF RUSSIAN ORIGIN

In accordance with Article 1p of Decision 2014/512/CFSP and Article 5t of Regulation (EU) No 833/2014, you are kindly requested to complete the below self-declaration to confirm whether or not you fall under the scope of the following:

a legal person, entity or body established in Russia that is publicly controlled or with over 50 % public ownership;

a legal person, entity or body established outside the European Union whose proprietary rights are directly or indirectly owned for more than 50 % by either the Russian government or a legal person, entity or body established in Russia that is publicly controlled or with over 50 % public ownership; or

a natural or legal person, entity or body acting on behalf or at the direction of either: (i) the Russian government; (ii) a legal person, entity or body established in Russia that is publicly controlled or with over 50 % public ownership; or (iii) a legal person, entity or body established outside the European Union whose proprietary rights are directly or indirectly owned for more than 50 % by either the Russian government or a legal person, entity or body established in Russia that is publicly controlled or with over 50 % public ownership.
